Your health records at Centric Health are the property of the practice; however, as a patient you are entitled to have a copy for your records or to send to other healthcare physicians.

If you are looking for records to send to another physician or for yourself, you must complete the Records Release Out form provided below. This completed form can be mailed or brought to the front desk of the office to be processed. It may take up to 10 days for your request to be processed.

There is a fee associated in providing copies of your records from our office. This fee is dependent on the number of pages and number of copies requested.
